Thank you for choosing Capitol Allergy & Asthma Center, we are looking forward to your visit. Your initial visit will include a comprehensive health evaluation that usually takes between 20-30 minutes. After an evaluation, we may perform additional tets that might include allergy skin testing, laboratory tests, office spirometry tests, and/or x-ray studies to arrive at an accurate diagnosis, to then design the most effective treatment for you.
Preparation for your Office Visit
You can save time in the office by completing your Patient Forms before your appointment.
Patients are seen by appointment only. Please call 24 hours in advance to cancel appointments you know you will miss.
Once you have called our office to schedule an appointment, there are a few things that you can do beforehand to make your visit go smoothly.
- Print and complete Our New Patient Registration Form and our Patient Acknowledgment Form, then bring them to our office
- Bring your primary insurance card and any secondary insurance cards
- Bring your driver's license or some other form of picture identification
- Bring a complete list of your medications and physicians (with contact information)
- Do not take any anti-histamines (i.e. benedryl, claritin, zytec, etc.) before your scheduled appointment (if possible). Please discuss any concerns with our medical staff before your visit.
- Please call the office if you are running late to your scheduled appointment or if you do not plan to keep you appointment
